Katie Duncan, an end-of-life nurse and a ‘death care coach’ who has seen many passings during her career, offers insights into what occurs as someone approaches death.
Duncan has experience in intensive care units, home hospices, and long-term care facilities. Throughout her years of service, she has observed patterns in her patients that she finds both mysterious and profoundly moving.
Her TikTok videos have gained significant popularity, and in her latest ones, she discussed a “mystical” occurrence that is involved in the dying process.

Duncan noticed that as individuals depart from this life, numerous patients tend to reach upwards, as if attempting to grab something above them.
“From what I’ve seen, this is quite frequent,” she mentioned in the video. “You can actually observe someone reaching up, as if they are trying to hold someone’s hand or connect with something in the atmosphere.”
These gestures are occasionally paired with what she refers to as end-of-life visions.

“People frequently mention that they see someone—a loved one, a pet, or even an angel or a bright light,” she shared. “At other times, they may not say anything, yet they still reach out, as if they can feel a presence.”
During these moments, the patients are not in distress. In fact, they appear to be quite relaxed.
“If anything, they seem to find comfort,” she remarked. “These visions, regardless of their source, often provide individuals with a profound sense of tranquility.”
Duncan adds, “It’s one of the more enigmatic elements of death that we are fortunate to observe as caregivers. It’s a humbling experience.”
Just like in her other videos, viewers responded to this one as well, recounting their own stories of loved ones performing similar gestures as they approached death.
“My father raised both arms. Before this moment, he was unable to lift them at all,” one individual recounted.
“When I saw this, I felt it was important not to interrupt him. It was such a meaningful moment,” another person remarked.
“This is the space in between. What a gift it is to witness,” a third commenter expressed.
